2016 Profile Instrument & Codebook

The 2016 Profile instrument was designed as a core questionnaire plus two separate modules. The core questionnaire was sent to every local health department (LHD) in the United States. Each module was sent to a random sample (stratified by population served). No LHD received more than one module.
